To request an urgent appointment for today or tomorrow (Monday to Friday)
- use askmyGP and select the most appropriate option for your medical concern (Mental Health / Urgent GP/ANP) Monday to Friday from 6am to 10am. We will respond during our opening times. (Please note, for patient safety the service may need to be turned off early)
- phone us on 01427 613 158, Monday to Friday, when we are open
- visit the surgery and speak with a receptionist, Monday to Friday, when we are open
When you get in touch, we’ll ask you to provide some details on your symptoms.
We will use the information you give us to signpost you to the most appropriate point of care.

Routine and follow-up appointments
To request a routine appointment (Monday to Friday or extended access):
- use askmyGP and select 'Routine' or 'Follow up GP/ANP' for a medical issue Monday to Friday 6am to 10am. We will respond during our opening times. (Please note, for patient safety the service may need to be turned off early)

However you choose to contact us we may offer you a consultation:
- by phone
- face to face at the surgery
- on a video call
- by digital correspondence.
- Ask you to attend a different provider which is more appropriate for your symptoms

Primary Care Networks (PCNs) provide enhanced access appointments between the hours of 6.30 pm to 8pm Mondays to Fridays and 9am to 5pm on Saturdays. As enhanced access will be delivered at a PCN level, not every practice has to be open during these hours.
We are pleased to be able to offer our patients, as part of Trent Care Network PCN, access to routine, pre-booked appointments at evenings and weekends. These are referred to as Enhanced Access appointments. The service is being delivered at the Cleveland Surgery, Gainsborough on behalf of the PCN and is available for any patient who is registered at any of the practices below:

Enhanced Access hours are between 6.30pm-8pm weekdays and 9am-5pm on Saturdays.
Appointments are for pre-bookable, non-urgent consultations (e.g., chronic illness, asthma checks, and blood pressure checks) with GPs, Nurses and Health Care Assistants. Please note, the enhanced access service is not a walk-in service.
As your registered practice (including Cleveland Surgery) will not be physically open during these hours, patients will be unable to have general queries answered or dealt with during enhanced access opening times. Please be aware, your appointment may be a telephone call, video call or a face-to-face appointment, you will be advised at the time of booking.
When booking an appointment, you will need to consent to a Consulting Clinician that may not be your own GP viewing and updating your health record.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
Please tell us:
- If there’s a specific doctor, nurse or other health professional you would prefer to respond.
- If you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email.
- If you need an interpreter.
- If you have any other access or communication needs.
